Forms and downloads

Every form the Ministry and its boards use with the public, in one place. Download the form, complete it in ink, attach what it asks for and lodge it where the form says. No form on this page costs anything.

How the forms are organised

Forms are grouped by who normally uses them: parents and pupils, head teachers and schools, teachers and other staff, and proprietors of private, community and mission schools. Each form carries a code in the pattern MOE/UNIT/NN — the middle segment names the unit that owns the form, so MOE/ADM is admission, MOE/CEN is the annual school census and MOE/APP is private-school approval. Quote that code in any correspondence about a form and the office will know immediately what you are asking about.

A form is only current if it carries the code and the revision date printed in the footer. Schools sometimes keep a stock of superseded forms in a cupboard for years; if the code on the paper in front of you does not match the code on this page, the Ministry will return it. Where a form must be signed by a head teacher, an Education Secretary or a Director, the signature must be original and the school stamp must be impressed over it.

Most forms may be lodged at the Local Government Education Authority office in the area where the school sits. The forty-four office addresses, telephone numbers and Education Secretaries are published in the office directory. Forms that create a legal status — a new school approval, a change of proprietorship, a teacher licence — are lodged at the responsible board headquarters and nowhere else.

Every form here is free

No officer of the Ministry, of a board, of a Zonal Education Office or of a Local Government Education Authority may charge you for a form, for a form to be stamped, for a form to be collected or for a form to be moved from one desk to another. Where a statutory fee is payable — an examination entry, a private school approval — it is published on the relevant service page and is paid to the Ministry, never to an individual.

If anyone asks you for money to release a form, note the office and the date and report it on 0800 MOE KANO or through /contact/feedback.

For parents and pupils

For head teachers and schools

Statutory returns. Where a return has a deadline, a late return is reported to the Education Secretary and appears on the school inspection record.

Completing and lodging a form

The same five steps apply to every form on this page.

Download the right version

Check that the code and revision date in the footer match what is published here. A superseded form is returned unactioned, and the delay is counted against you, not against the office.

Complete it in block capitals, in ink

Every field must be filled. Where a field does not apply, write "N/A" rather than leaving it blank — a blank field is treated as an omission and sends the form back. Do not use correcting fluid; strike through, initial and rewrite.

Attach the documents the form lists

Attach photocopies, not originals, unless the form says otherwise, and bring the originals for sighting. Every photocopy must be certified by the head teacher, an Education Secretary or a commissioner for oaths.

Have it signed and stamped

A form that passes through a school must carry the head teacher or principal signature with the school stamp impressed over it. A form from a private school must carry the proprietor signature and the school approval number.

Lodge it and collect an acknowledgement

Lodge the form at the office named on its last page and insist on a stamped acknowledgement slip showing the date received. That slip is what you quote when you follow up, and it is what starts the clock on the published processing time.

Why forms are sent back

These nine faults account for most of the forms the Ministry returns unactioned. Check for them before you leave the school.

  • An out-of-date revision of the form, usually taken from a school cupboard
  • A blank field where "N/A" should have been written
  • No school stamp, or a stamp that does not overlap the signature
  • Photocopies that nobody has certified
  • A pupil name spelt differently from the way it appears on the birth certificate
  • No telephone number, or a number that has been out of service for months
  • A private school approval number quoted for an approval that has expired
  • A census or enrolment return that does not separate boys from girls
  • No acknowledgement slip collected, so there is no proof the form was ever lodged

Questions about forms

I do not have a printer. Can I still get a form?
Yes. Every form on this page is stocked in hard copy at the Local Government Education Authority office in each of the forty-four local government areas, and at the six Zonal Education Offices. Ask at the counter, quote the form code, and the officer on duty will issue you a copy at no charge.
Can I lodge a form on behalf of someone else?
You may lodge a form for a member of your household or for a pupil in your care. For anything that creates or changes a legal status — a school approval, a change of proprietorship, a teacher licence — the applicant must attend in person with identification, or send a representative carrying a letter of authority and a copy of the applicant identification.
How long does the office keep a lodged form?
The lodging office keeps the paper file for the life of the matter and then transfers it to the records room of the responsible board. Enrolment, census and results returns are retained permanently because they are the basis of the published education statistics. A pupil record is retained for the whole of the pupil school career and for twenty-five years afterwards.
A form asks for my school code. Where do I find it?
The school code is printed on the school approval or establishment certificate and appears in the published school register at /schools/directory. It takes the pattern KN/LGA/CATEGORY/NNNN — for example KN/MUN/SSS/0142. If a school genuinely has no code, the Education Secretary applies for one on the school behalf.
Is there an electronic version of any of this?
Admission, transfer certificate, scholarship, bursary, textbook requisition, school approval and teacher licensing applications can all be started online from the e-services pages. Statutory returns from schools are lodged through the school portal by the head teacher. The paper forms remain valid and are still accepted at every counter.
What if the office refuses to accept my form?
Ask for the reason in writing on the acknowledgement slip. If none is given, take the form to the Zonal Education Office for the area, or telephone the hotline. A form that meets the requirements printed on its own last page must be received; an officer may query it afterwards, but may not refuse to take it.
